Adult Library Card

Applications are available at the circulation desk or here. You must present proof of residency in the Locust Valley Library district.

Please note: The Locust Valley Library district follows the boundaries of the Locust Valley CSD # 3 – 4, the Locust Valley portion of the school district only. The area that was served by the former Brookville Elementary School District (which includes the parts of Brookville, Matinecock, Muttontown, Old Brookville and Upper Brookville that are in the Locust Valley School District) is not in the Locust Valley Library district. Residents of this area cannot be issued a Locust Valley Library card. They are eligible to obtain a State Mandated Access Card (SMAC card) at either the East Meadow Public Library or the Levittown Public Library.

Library card applicants must present a valid photo identification, such as:
*Driver license
*State-issued identification card
*Passport
*Employee identification card

When is a driver license not enough? If the street address is not listed on the photo ID, a second item listing street address and the applicant’s name must also be provided. Please see our Library Card Acceptable Proofs Policy for more information. 

Children’s Library Card

Children must be able to sign their name and have parental approval. Please bring in one of the above mentioned examples of proof of residency.
